Deputies of the Logan County Sheriff’s Office and Bellefontaine EMS responded about 4:35 a.m. Thursday to a rollover crash on State Route 47 in front of the Bellefontaine Airport, 3700 W. S.R. 47.
Driver Joshua D. Green, 29, of East Liberty, reportedly refused transported from the scene. He told deputies he had swerved to avoid a deer in the road and his westbound pickup went left of center off the south side of the road and continued westbound along the ditch. He then re-entered the road and his vehicle began to overturn. His vehicle then went off the north side of the road and into a cornfield, coming to rest on its back tires.
Damage was heavy to the pickup, which was towed from the scene. Green was cited for failure to control and the crash report indicates he was not wearing his seat belt.
• Drivers Justin H. Mullins, 34, of Lakeview, and Joshua R. Spore, 43, of Nevada, Ohio, each were checked at the scene by Indian Lake EMS, but were not transported following a crash Thursday afternoon.
Deputies report Mullins was operating a southbound sport utility vehicle on State Route 235 about 5:25 p.m. and struck the rear of a southbound minivan operated by Spore that was stopped for traffic waiting to turn into the Indian Lake State Park Campgrounds, 13156 S.R. 235, Lakeview.
Damage was heavy to the SUV and moderate to the minivan. Mullins was cited for failure to maintain assured clear distance ahead. He was wearing his seat belt and
Spore was wearing a shoulder belt only.
Forgery report involving doctor’s note investigated
Deputies received a report Thursday from Bellefontaine Police Department officers regarding an employee at a Honda of America at East Liberty presenting a forged doctor’s note at his workplace regarding a face mask requirement.
Officers had contacted the doctor’s office in Bellefontaine, and the employees indicated that the male suspect had not been seen at the office recently.
Deputies spoke with the suspect, who admitted that he had written the doctor’s note himself at home, indicating a recommendation for him to wear a face shield at work instead of a face mask. He also said he had used a word processing program to “sign” the doctor’s name on the letter.
The case has been forwarded to the Logan County Prosecutor’s Office for review and determination of potential forgery charge.
• Deputies took a fraud report Thursday from a Zanesfield woman, who said she had been notified that her SIM card was transferred by her cell phone carrier to a new phone at a store in New Jersey.
Shortly after that time, she observed two fraudulent charges on her checking account in the amounts of $4,000 and $4,500. She said a female suspect had used a fake ID to make the withdrawals at a New Jersey bank. In addition to contacting her bank, she also was advised to report the incident to the Ohio Attorney General’s Office.
